Story of
Alex Matter
A 22-year-old youth with an intellectual disability and PTSD had a difficult and depressing life until he met Legacy Care Solutions.
He lived alone in a unit rented by government housing and went to the homeless shelters to get food. He is now a part of the Centrelink Job Seeker Program and receives a youth allowance.
Affected by intellectual disability, childhood trauma, ADHD and depression, Alex had difficulties handling his emotions and lacked life skills.
Here is how we’ve helped Alex.
He was referred to us by NDIS to establish and coordinate his 1st NDIS plan.
NDIS set the following goals we at Legacy Care Solutions helped him achieve.
- Further developing his life skills.
- Getting him to learn positive strategies to regulate his emotions.
We took a lot of preparation for the first plan meeting. We engaged a Support Coordinator to explain the new plan to him and connect him with an efficient provider according to his NDIS plan.
The first plan meeting was held in person at a cafe, and it went on for two hours. The meeting went smoothly as we planned it well.
Within 2 weeks of the meeting, our Support Coordination team confirmed to Alex that all support would be established according to his goals and personal preferences.
After the meeting, our Support Coordinator collected all his relevant documents and assessments from the existing stakeholder. In addition, the following assessments were also done for Alex-
- Participant Risk Assessment
- Home Environment Assessment
- Initial Assessment and Support
The Support Coordinator has done the following tasks according to his NDIS plan and fund:
- Established Alex's service with an Occupational Therapist, a Psychologist and a Behaviour Therapist who provide their services in areas close to his home to ensure convenience.
- The Support Coordinator accompanied Alex to all initial assessments with the therapists to ensure that Alex did not miss any points and received the best outcomes.
- Engaged Alex with a Support Worker to take him shopping, to the café and the beach area. The Support Worker was also assigned to help Alex with cooking at home to build up his cooking capacity.
- The Support Coordinator is now working with him to get him back to school and start a course (TAFE).
- The Support Coordinator explained the NDIS Price Guide for his services to him and also advised him on how he can avail maximum service with the existing fund.
- The Support Coordinator is also helping him to connect with other Government Disability Support Services such as job search and how to start a career.
- The Support Coordinator also sends the initial progress report to NDIS according to his Plan requirement.
With these measures taken and due to receiving the continuous support provided by the Legacy Care Support Coordinators, Alex’s life has become much more manageable. We are all working together to give him an even better future where he lives a more independent and happier life.
